TO REGISTER WITH CBAC:

Please fill out the following forms (forms can be downloaded from this page or you can get them from Yamile Hewitt 301-863-1334). You can click on the Registration Form, Medical Release Form, and USA SWIMMING Form links below to print the forms. Note: you may type information into the forms and print them. Level and fees for new members will be determined during a swim assessment. View fees by group.

Forms:

To access the online forms, you must have Acrobat Reader installed on your computer. Click the Acrobat Reader icon to go to the Adobe website and download Acrobat Reader.

1. CBAC Registration Form - Have the coach assign a level for your child and write it on Line 18. Please contact Yamile Hewitt 301-863-1334 so she can tell you what the cost is for that level (pro-rated throughout the season). Payment must be made by check or cash. Checks should be written out to CBAC. Click here to download the CBAC registration form.
2. Medical Release Form - Fill this out with all the information listed. Download the medical release form.
3. 2008 USA Swimming Athlete Registration Form - Please print this form and fill it out once per year. The club code is CBAC and the name of the club is the Chesapeake Bay Aquatic Club. Please make sure that you sign the bottom before returning this to Yamile. Note: The check for this fee also needs to be written out to CBAC. 2008 USA Swimming fees are $64.00.
4. 2008 USA Swimming Non-Athlete Registration Form - CBAC Board Members and Safety Offiicals must fill out this form. Please print this form and fill it out once per year. The club code is CBAC and the name of the club is the Chesapeake Bay Aquatic Club. Please make sure that you sign the bottom before returning this to Yamile.

6. RETURN all the forms to Yamile Hewitt or put in the bright yellow REGISTRATION folder. Yamile will place a folder with your name on it in the file boxes. This folder will be for communication between the coach, Board Members, and Yamile. It is very important that you check it when you come to the pool.
7. Please be sure to subscribe to the CBAC Swimming yahoo group to get important email alerts and reminders.

Registration Notes:

• See Coach Chuck for a swim assessment to determine the proper level for your swimmer
• USA Fees are paid only once per year (covers insurance among other things)
• Swimmers registering with CBAC for the first time must bring a copy of their birth certificate (USA Swimming rule)
• Fee for 3rd swimmer in family is 1/2 price
• 4th swimmer in family swims for free
